PHOENIX BEHAVIORAL HEALTH SERVICES
What is Phoenix?
Phoenix is a day program that serves the needs of Lakeside residents who require special services for mental health and behavioral needs. The program is held from 930am to 230pm Monday-Friday. We offer comprehensive care from a team of dedicated mental health professionals.
Who is eligible for the program?
o Current residents of Lakeside Nursing and Rehabilitation Center
o Persons from the community who are referred by family or friends
o Persons who are referred by other agencies, including but not limited to other nursing facilities, assisted living facilities, Adult Homes, Adult Protective Services, Acute Psychiatric Centers.
o All referrals will be reviewed by both the Phoenix team and by our Director of Nursing to determine appropriateness for Lakeside and the Phoenix program

o Individual and group psychotherapies
o Music therapy
o Therapeutic arts and activities
o Psychiatric consult
o Psychological consult

Phoenix is proud to have celebrated our one year anniversary in March of 2008. New and exciting program elements have been added, and positive results have been observed. Residents, staff, and family have expressed appreciation for the Phoenix program.

She remarked that, "The different elements of therapy:
music therapy, group therapy, individual therapy,
intergenerational programming, creative expression
group, and support groups have been like opening a
closed door to a new world." She went on to say,
"The Phoenix program has been helpful as a human
being to appreciate being able to be seen and see
others as an individual, not as their physical or
behavioral challenges."
