Rehabilitation Services Our "StoneBridge" rehabilitation unit focuses on recognizing and resolving each resident's post-acute needs, and bringing their successful return home. StoneBridge services
include: Physical Therapy - Physical Therapy staff work together with doctors and nursing staff to provide prescribed intensive rehabilitative programs onsite for residents.
Occupational Therapy - Our therapists help individuals gain strength and independence in fine motor control and daily living activities.
Speech Language Therapy
- Lakeside provides
speech and
language evaluation, swallowing
evaluation, therapy, and
treatment.

Our cheery,
social environment for physical therapy encourages
residents to work harder and longer than they thought
they could. (Residents may choose a less social approach
if they prefer it.) Many go home much sooner than they
thought possible-but come back to
visit.

Transitional Care Hospital stays are becoming ever shorter. Many people are seeking round-the-clock transitional care for themselves or for their family members.
Lakeside offers a convenient and
safe place to rest, recuperate, and regain strength
after a hospital stay, before going home with another
family member, or to self-care.

Respite Care Caring even
for someone you love dearly in the home, 24 hours a day, can be exhausting. Home
caregivers may take a much-needed break for a vacation, attend to
out-of-town-business or other family functions, or just
to relax, knowing that the staff at Lakeside will
provide a safe short-term home for a loved one for a few days or a few
weeks.

Skin and Wound Care Prevention
of skin wounds is primary at Lakeside. The Wound Care
multidisciplinary team involves physical therapists,
dietitians, and nurses. Chief among their goals is
prevention. Our
mattresses are state-of-the-art. Enormous attention is given to
repositioning and moving residents who can notinitiate turning and
positioning on their
own.

Attention
is also paid to a diet that helps prevent skin
deficiencies, and to the assistance with personal
hygiene of residents who are unable to care for themselves.
Wound healing processes are carefully monitored. The team is
confident of their ability to work with the toughest
wounds.While many nursing homes shy away from accepting
residents in need of wound care, Lakeside welcomes rather than
rejects residents with major wounds.
Hospice Services Our
Lakeside team is well-trained and sensitive to
end-of-life issues. It is our goal to make every moment
a person spends with us as comfortable and pleasant as
possible. To achieve this, our team works closely with
families on issues like feeding, hydration, DNR's, and
of course, simply being with the person who is dying. Clergy and social
workers are available for support when
needed. |

In
addition, eligible residents and their families may
elect the Hospice Benefit. Lakeside has formed a
partnership in care with Hospicare and Palliative Care
Services of Tompkins County. Our teams work closely
together, providing additional ongoing training and
information to each other in our specific areas of
expertise.

Dementia Care Our secure dementia unit, SageHeritage,
is so named in recognition of the wisdom and knowledge
held and contributed by each resident that link our
pasts to the present. SageHeritage was created to assist
individuals with memory impairments such as Alzheimer’s
and other forms of dementia. Specialized programming has
been created to stimulate our residents throughout the
day. Our specially trained staff encourage our residents
to participate in the most current therapies,
activities, and services available.
